Recruitment consent (version 2)

alternative to !2338 (closed). This is not a fundamentally different approach. I just didn't want to mess around with @hayat's commits, so I created a fresh branch.

The goals are:

• get rid of the waiting/confirmed distinction
• allow users to explicitly select a document
