rm subject notes

The hardcoded notes are barely used (0.2%, 0.2%, 0.6%). The free-text notes are used more often (2.5%) but mostly for things that would better be covered by other features such as availability, study type disinterest, or study participation.

The UI is not great either and formsets are always complicated. So I propose to simply remove the whole feature.
